Can I Take Levothyroxine With Wegovy Tablets?

If you take levothyroxine for an underactive thyroid and are considering Wegovy tablets, you may immediately spot a practical problem:

Both medicines have specific instructions about taking them on an empty stomach.

So can you take levothyroxine and Wegovy tablets together?

Yes,  but timing matters

Wegovy tablets contain oral semaglutide and should be taken after fasting for at least 8 hours.

Take your Wegovy tablet with up to around 120ml of water, then wait at least 30 minutes before eating, drinking or taking another oral medicine.

Levothyroxine also has specific instructions. NHS guidance recommends taking it on an empty stomach, usually 30–60 minutes before your first meal, caffeinated drink or other medicine.

That means simply swallowing your Wegovy and levothyroxine tablets together isn't recommended.

Does Wegovy interact with levothyroxine?

There is a recognised interaction.

Studies have found that taking oral semaglutide at the same time as levothyroxine can make your body absorb more levothyroxine than usual, about one-third more overall (33%).

Because of this, the manufacturer advises that monitoring of thyroid parameters should be considered when semaglutide and levothyroxine are used together.

This does not mean the medicines cannot be used together.

It means your prescriber should know that you take levothyroxine and may wish to consider your dosing routine and thyroid blood-test monitoring.

What could my morning routine look like?

One possible routine, if suitable for you and agreed with your healthcare professional, might be:

Wake up → take Wegovy → wait at least 30 minutes → take levothyroxine → then follow your usual levothyroxine fasting instructions before breakfast or coffee.

Because levothyroxine dosing and thyroid control vary between patients, don't change your established dosing routine without discussing it with your doctor, pharmacist or prescriber.

For someone taking several morning medicines, a daily tablet with fasting requirements may not necessarily be more convenient than a once-weekly injectable treatment.
