- Wegovy 25mg Tablets Not Working? What...

You've reached Wegovy 25mg tablets.
You're taking the maximum recommended daily dose but suddenly the scales aren't moving.
So has Wegovy stopped working?
Not necessarily.
First: a plateau doesn't always mean treatment failure
Weight loss rarely happens in a perfectly straight line.
As you become lighter, your body's energy requirements also change. You may therefore find that the calorie intake which originally produced weight loss eventually becomes closer to your new maintenance requirements.
A few days or even a couple of weeks without movement on the scales isn't necessarily a true plateau.
Look at the longer-term trend, rather than individual weigh-ins.
Check how you're taking your Wegovy tablet
Before deciding that 25mg isn't working, this is particularly important.
Oral semaglutide has low and variable absorption. The SmPC estimates that around 1–2% of the oral dose is absorbed, and taking it with food or large quantities of water reduces absorption further.
Take your tablet:
after an 8-hour fast → with up to 120ml water → wait at least 30 minutes before anything else.
The manufacturer specifically advises clinicians to check adherence to this routine when treatment response is lower than expected.
How long have you been on 25mg?
Don't judge the maintenance dose from only a few days.
Oral semaglutide has a half-life of approximately one week, with steady-state concentrations reached after around 4–5 weeks of once-daily dosing.
Your prescriber can therefore consider both how long you've been at your current dose and your wider treatment history.
How much weight do people lose on Wegovy 25mg?
In the OASIS 4 study, participants receiving oral semaglutide 25mg experienced an estimated average 13.6% reduction in body weight at 64 weeks, compared with 2.2% with placebo under the main treatment-policy analysis.
But averages aren't guarantees.
Some patients lose more, others less.
And importantly, your aim may eventually become weight maintenance rather than further loss.
Should I take more than 25mg?
No.
25mg is the maximum recommended single daily oral Wegovy dose.
Do not take additional tablets or combine lower strengths in an attempt to increase the effect.
What should I do if my weight really has plateaued?
This is the point for a proper treatment review.
At Pharmulous, we'd look at factors such as:
appetite control, tablet administration, nutrition, activity, weight trend, side effects and your actual goal weight.
Sometimes continuing treatment is appropriate. Sometimes the issue is maintenance rather than failure. In other cases, your clinician may need to consider whether a different strategy is suitable.
A plateau should trigger a review, not panic.
