- Wegovy 4mg Tablets: What to Expect When...

If you have completed your first month of Wegovy 1.5mg tablets, the next step in the usual dosing schedule is Wegovy 4mg once daily.
This is the first increase in your oral Wegovy treatment and understandably raises plenty of questions.
Will you feel a bigger difference? Will you lose more weight? Could side effects return? And what happens if 4mg is already working well for you?
At Pharmulous, we believe dose increases should always be viewed in the context of the individual patient, not simply as a race towards the highest dose.
What is Wegovy 4mg?
Wegovy tablets contain semaglutide, a GLP-1 receptor agonist used for weight management alongside a reduced-calorie diet and increased physical activity.
The licensed oral Wegovy schedule starts at 1.5mg once daily for at least one month, followed by 4mg, 9mg and eventually 25mg, with a minimum of one month at each dose level. Importantly, the UK product information also states that the dose can be maintained at the previous level if needed.
For most people following the standard schedule, 4mg therefore represents month two of treatment.
What is the purpose of increasing from 1.5mg to 4mg?
The gradual increase is designed to allow your body time to become accustomed to semaglutide.
The starting dose is primarily the first step in this process. Moving to 4mg increases your exposure to the medicine while continuing the gradual escalation intended to improve tolerability.
Some people may begin noticing greater appetite control, earlier fullness or less interest in food as treatment progresses. Others may have noticed these effects from the starting dose already.
There is no single response that everyone should expect.
How much weight can I lose on Wegovy 4mg?
This is one of the most common questions but there is no reliable number of kilograms or percentage of body weight that somebody should expect to lose specifically on 4mg.
The major clinical study supporting oral Wegovy for weight management evaluated patients through a progressive dose-escalation programme towards 25mg rather than establishing a separate expected weight-loss figure for every individual dose.
Weight loss also depends on factors such as your starting weight, diet, physical activity, appetite response, other medical conditions and how consistently the medicine is taken.
So don't be discouraged if somebody online says they lost more weight during their second month than you have.
Your response is your response.
Will Wegovy 4mg reduce my appetite more?
It may.
Semaglutide acts on GLP-1 receptors involved in the regulation of appetite and calorie intake.
As the dose increases, some patients report feeling fuller sooner, thinking about food less frequently or finding portions naturally becoming smaller.
But the goal is not to completely eliminate hunger.
Hunger is a normal physiological signal. Successful weight management is about gaining better control over appetite and eating behaviour rather than being unable to eat.
What side effects can happen when increasing to Wegovy 4mg?
Some patients experience very few side effects.
Others may notice gastrointestinal symptoms when the dose changes, including nausea, constipation, diarrhoea, vomiting, indigestion or abdominal discomfort.
The Wegovy product information specifically notes that gastrointestinal effects are reported most frequently during the dose-escalation period, and in the OASIS 4 study most nausea, vomiting and diarrhoea events were mild to moderate and generally short-lived.
That means it is possible to feel perfectly comfortable on 1.5mg and temporarily notice symptoms again after moving to 4mg.
How can I make the step up easier?
Keep meals sensible in size, eat slowly and pay attention to when you begin feeling full.
Very large, rich or fatty meals can be more difficult to tolerate for some people taking GLP-1 medicines.
Hydration is also important, particularly if you develop vomiting or diarrhoea, because gastrointestinal side effects can contribute to dehydration.
If side effects are persistent, significant or interfering with normal eating and drinking, contact the Pharmulous clinical team rather than simply struggling through them.
How should I take Wegovy 4mg tablets?
The way oral Wegovy is taken is important because food and timing can affect semaglutide absorption.
Take one 4mg tablet once daily after fasting for at least 8 hours.
Swallow the tablet whole with a small amount of water up to approximately 120mL.
Then wait at least 30 minutes before eating, drinking anything else or taking other oral medicines.
Do not crush, split or chew the tablet, and do not take multiple tablets in an attempt to create a higher Wegovy dose.
What if I forget my 4mg Wegovy tablet?
If you miss a dose, skip the missed tablet and take your normal dose the following day.
Do not double up to compensate.
Do I automatically have to increase to Wegovy 9mg?
The standard licensed schedule progresses from 4mg to 9mg after at least one month.
However, the Summary of Product Characteristics also states that, if needed, the dose can be maintained at the previous dose level.
That is an important principle at Pharmulous.
We don't believe every patient's treatment should simply become:
“Month finished — dose up.”
Your weight trend, appetite, side effects, nutrition, tolerability and overall clinical response matter.
What if 4mg is already working really well?
That's something worth discussing with your prescriber.
More medicine is not automatically better medicine.
If you are experiencing meaningful appetite control and appropriate weight management without troublesome side effects, Pharmulous takes a lowest-effective-dose approach where clinically appropriate.
Equally, if appetite control is inadequate and the medicine is well tolerated, progressing through the licensed schedule may be appropriate.
The decision should be clinical rather than automatic.
What if I don't feel anything on Wegovy 4mg?
Don't panic.
Response to semaglutide varies considerably between individuals, and 4mg is still an early stage of the licensed escalation schedule.
Not feeling dramatic appetite suppression does not necessarily mean the medicine will not work for you.
Your clinician can assess your overall response as treatment progresses.
How Pharmulous can help
Weight management isn't just about getting through dose increases.
At Pharmulous, we look at whether the treatment is actually working for you.
That means considering your appetite, food choices, weight trend, side effects, lifestyle, previous treatment and longer-term objectives.
We are doctor-led, maintenance-friendly and focused on the lowest effective dose where clinically appropriate.
Whether you are losing weight, approaching your target or already thinking about maintenance, treatment should fit the patient — not the other way around.
Frequently Asked Questions
Is Wegovy 4mg a high dose?
No. It is normally the second stage of the oral Wegovy escalation schedule, following 1.5mg and before 9mg and 25mg.
How long do I take Wegovy 4mg?
The licensed schedule uses each dose for a minimum of one month before progressing. Your prescriber may decide that staying on a previous dose is appropriate if needed.
Can I stay on Wegovy 4mg?
Potentially, where clinically appropriate. The product information permits maintaining treatment at the previous dose level if needed.
Should I lose more weight on 4mg than 1.5mg?
Not necessarily. Individual weight loss cannot reliably be predicted dose by dose.
Can I take two Wegovy tablets to make a stronger dose?
No. Wegovy tablets should be taken as one tablet per day, and multiple tablets should not be combined to create a higher dose.
