- Can Cialis Affect Your Eyes? New Research...

If you take tadalafil, commonly known by the brand name Cialis. for erectile dysfunction, you may have seen recent headlines linking the medicine with glaucoma.
The headlines sound worrying.
But the research needs putting into context.
A large study published in the British Journal of Ophthalmology found an association between long-term tadalafil use and a modestly higher rate of glaucoma. It did not prove that Cialis causes glaucoma, and the men studied were not simply occasional users taking tadalafil before sex.
What did the tadalafil study find?
Researchers examined health records from almost 74,000 men aged 40 and over.
They compared 36,927 men prescribed tadalafil with an equal number of similar men who had not used PDE5-inhibitor medicines.
Over five years:
- glaucoma occurred in 3.93% of tadalafil users
- compared with 3.16% of non-users.
After accounting for other factors, this represented a 22% higher relative risk of glaucoma.
Tadalafil users also showed higher rates of ocular hypertension and primary open-angle glaucoma.
The absolute difference is worth remembering: it was less than one percentage point over five years.
Does this mean Cialis causes glaucoma?
No.
This was an observational study.
Observational research can identify an association but cannot prove that one thing caused the other.
Researchers also relied on healthcare-record diagnoses and could not confirm precisely how much tadalafil every patient actually took throughout follow-up.
Further research is therefore needed before we know whether tadalafil itself increases glaucoma risk.
Does this apply to tadalafil used for erectile dysfunction?
This is another important limitation.
The study specifically examined men using tadalafil for lower urinary tract symptoms, commonly associated with an enlarged prostate.
That often involves regular, long-term tadalafil treatment.
Many men using tadalafil purely for erectile dysfunction instead take 10mg or 20mg when required, rather than every day.
We therefore cannot assume that occasional ED treatment carries the same risk found in this study.
Are eye problems already recognised with tadalafil?
Certain rare eye problems are already recognised in UK tadalafil product information.
These include non-arteritic anterior ischaemic optic neuropathy (NAION) and retinal vascular problems. Anyone experiencing sudden loss or significant change in vision while taking tadalafil should stop treatment and seek urgent medical advice.
Glaucoma itself has not currently been added as an established tadalafil adverse effect or contraindication in the UK product information I reviewed.
The new study should therefore be regarded as a potential safety signal requiring further investigation, rather than proof of a new side effect.
What if I already have glaucoma?
Don't automatically stop prescribed tadalafil because of this study.
However, if you have glaucoma, raised eye pressure, severe short-sightedness or a strong family history of glaucoma, it is sensible to make sure your prescriber knows.
The study authors suggested that eye assessment and follow-up may deserve consideration particularly in people receiving long-term tadalafil who already have glaucoma risk factors.
How Pharmulous can help
Erectile dysfunction treatment should never simply be about selecting a tablet.
At Pharmulous, treatment requests are reviewed by UK-registered clinicians who consider your medical history, current medicines, cardiovascular health and relevant risk factors before deciding whether treatment is suitable.
If you are already using tadalafil and have concerns about side effects, or you're unsure which ED treatment may be appropriate for you, you can complete a confidential Pharmulous men's-health consultation for individual clinical assessment.
