- My NHS Wegovy Is Ending After 2 Years...

If you've successfully lost weight with Wegovy through the NHS, you may now be facing an unexpected question:
What happens when my two years of treatment ends?
This is becoming increasingly relevant as some of the first UK patients prescribed Wegovy through specialist NHS weight-management services approach the end of their treatment period.
For people who have worked hard to lose weight and improve their health, the prospect of suddenly losing that support can understandably be concerning.
Why does NHS Wegovy stop after two years?
Current NICE guidance for semaglutide used for weight management under TA875 recommends treatment for a maximum of two years within a specialist weight-management service.
Importantly, this does not mean that Wegovy suddenly becomes unsafe after two years.
NICE's original appraisal explains that the restriction reflected the evidence and cost-effectiveness modelling available at the time, as well as the way specialist NHS weight-management services were structured. NICE itself acknowledged that obesity is a long-term condition and that stopping effective treatment can be challenging.
The current UK Wegovy product information describes ongoing maintenance dosing and individual review if adequate weight loss has not been achieved, rather than imposing a universal two-year medicine-licensing cut-off.
Will I regain weight when Wegovy stops?
Not everybody responds in exactly the same way, but weight regain after stopping GLP-1 treatment is common.
A major 2026 systematic review led by University of Cambridge researchers examined 48 studies of GLP-1 receptor agonist discontinuation.
Among the randomised trials included in their modelling, approximately 60% of the weight previously lost was regained within one year of stopping treatment.
The researchers estimated that regain may eventually plateau below the original starting weight, but the longer-term figures were modelled projections rather than direct long-term observations, so they should not be treated as a certainty.
The important message is simpler:
Reaching your target weight doesn't necessarily remove the biological factors that contributed to weight gain in the first place.
Does that mean I need Wegovy forever?
Not necessarily.
Weight maintenance should be individualised.
For some people, continuing pharmacological treatment may remain clinically appropriate. Others may reduce treatment, change their maintenance strategy or eventually stop medication with appropriate support.
The right decision depends on factors including your weight history, current BMI, amount of weight lost, appetite, medical conditions, treatment response, side effects and risk of weight regain.
There is no single maintenance plan that suits everyone.
Can I continue treatment privately after NHS Wegovy ends?
Potentially, yes but this requires a new clinical assessment.
Someone transferring from established NHS treatment should not necessarily be assessed in exactly the same way as somebody who has never used weight-management medication.
A private clinician can consider your original weight and BMI, treatment history, current dose, amount of weight lost, current weight, tolerability and ongoing need for weight maintenance when deciding whether continued treatment is clinically appropriate.
Treatment should never be automatically continued simply because it was previously prescribed.
Don't wait until your final Wegovy dose
If you know your NHS treatment is approaching its end, it makes sense to think about your maintenance strategy before the final prescription runs out.
That allows time to review your treatment history and consider whether continued medication, a dose adjustment, an alternative treatment strategy or supported discontinuation is appropriate.
Avoid altering your dose, stretching injections or experimenting with so-called “microdosing” without clinical guidance.
How Pharmulous can help
Pharmulous has a dedicated Weight Maintenance service for people who have reached or are approaching their target weight, including patients who began treatment elsewhere and want their ongoing care reviewed.
Our clinicians consider your full weight-loss journey rather than looking only at today's BMI and aim to identify the lowest clinically appropriate treatment strategy capable of supporting stable, healthy weight maintenance where continued medication remains suitable.
If your NHS Wegovy treatment is approaching its end, you can complete a Pharmulous weight-maintenance assessment so that your options can be reviewed individually rather than waiting until treatment has already stopped.
