- Wegovy 7.2mg Explained: Who Is It For...

Until recently, 2.4 mg once weekly was the highest maintenance dose most Wegovy patients would encounter.
That changed in 2026.
The MHRA has now authorised semaglutide 7.2 mg once weekly as an additional Wegovy dose for selected adults with obesity. A dedicated single-dose 7.2 mg pen was subsequently approved in April 2026.
Do you start Wegovy at 7.2mg?
No.
The standard Wegovy escalation remains:
0.25 mg → 0.5 mg → 1 mg → 1.7 mg → 2.4 mg
with at least four weeks at each stage.
Only after at least four weeks on 2.4 mg may 7.2 mg be considered for an adult with obesity where clinically needed.
Why was a higher dose introduced?
Not everyone reaches their treatment goals on 2.4 mg.
The STEP UP trial therefore investigated whether a higher semaglutide dose could produce additional weight reduction.
At 72 weeks, the treatment-policy analysis showed average losses of 18.7% on 7.2 mg versus 15.6% on 2.4 mg.
Clinical-trial averages are not predictions for an individual patient. Some people lose considerably more, others less.
Does everyone eventually move to 7.2mg?
No.
This is one of the most important things to understand.
7.2 mg is an option, not a destination everyone should aim for.
A patient doing well on 1.7 mg or 2.4 mg does not need a higher dose simply because it exists.
Your clinician should consider benefit, side effects, weight trajectory and whether further weight reduction is appropriate.
Is the 7.2mg pen different?
Yes.
The dedicated 7.2 mg product is a single-dose pre-filled pen, delivering one weekly 7.2 mg dose.
This differs from the standard multi-dose FlexTouch pens used for lower Wegovy strengths.
What if 2.4mg is already working?
Then there may be no reason to change.
At Pharmulous, our approach is not based on reaching the maximum dose as quickly as possible.
The treatment plan should follow clinical response and tolerability, not simply the number printed on the pen.
How Pharmulous can help
If you are already taking Wegovy and are wondering whether 7.2 mg is relevant to you, Pharmulous can review your treatment history, current dose, weight trajectory, side effects and longer-term goals.
More medication is not automatically better medication.
The right dose is the one that provides appropriate benefit while remaining safe and tolerable.
