- Wegovy for Weight Maintenance: Do You...

Getting to your goal weight raises a surprisingly difficult question:
“What happens now?”
For many people, stopping treatment immediately and hoping appetite remains permanently changed isn't realistic.
What happens when semaglutide is stopped?
The STEP 1 extension followed participants after semaglutide treatment ended.
During the year after stopping, participants regained around two-thirds of the weight they had previously lost, on average.
That doesn't mean everyone regains weight.
But it demonstrates why obesity and weight management increasingly need to be considered as long-term conditions, rather than something that ends the day the scales reach a particular number.
Can Wegovy be used long term?
Clinical trials have studied semaglutide 2.4 mg over prolonged periods.
In STEP 5, participants treated for 104 weeks maintained substantial average weight loss compared with placebo.
STEP 4 also showed that people who continued semaglutide maintained and extended their weight reduction, whereas those switched to placebo regained weight.
Do I need the highest dose forever?
Not necessarily.
Maintenance isn't the same thing as continuing to chase further weight loss.
Your clinician needs to consider:
your stable weight, appetite, treatment response, side effects, medical history, relapse risk and whether your current dose continues to provide benefit.
Some patients need ongoing pharmacological support; others may eventually be able to adjust treatment.
The important part is planning rather than abruptly stopping without considering what happens next.
What if my BMI is now normal?
A lower BMI after successful treatment does not erase your treatment history.
Clinical decisions about continuing weight management should consider how you reached your current weight and the risk of regain, rather than simply treating your current BMI as though you had never had obesity or overweight.
How Pharmulous can help
Weight maintenance is a core part of the Pharmulous approach.
We understand that reaching a healthy weight is not necessarily the end of treatment.
Our clinicians can review whether continuing Wegovy, adjusting your dose or considering another maintenance strategy is appropriate based on your individual history.
Losing weight matters. Keeping it off matters too.
