Foundayo for Weight Maintenance After Mounjaro: Is It a Good Long-Term Option?

Reaching your goal weight on Mounjaro is a significant achievement. However, for many people, reaching their target is not the end of treatment - it marks the beginning of weight maintenance.
The question often changes from:
“How do I lose more weight?”
to:
“How do I keep the weight off?”
With the introduction of Foundayo (orforglipron), eligible patients now have another option to consider. Foundayo is a once daily oral GLP-1 tablet authorised in the UK for weight management, including both weight loss and weight maintenance.
For some people who have successfully lost weight with Mounjaro, Foundayo may therefore provide an alternative way of supporting longer-term maintenance.
Why does weight maintenance after Mounjaro matter?
Mounjaro can help reduce appetite, increase feelings of fullness and make it easier for some people to manage food intake.
After reducing or stopping treatment, some patients notice their hunger or food cravings gradually returning. Weight regain can also occur.
This does not mean treatment has failed. Weight management can require long-term support, and the treatment needed during maintenance may be different from that required during active weight loss.
During maintenance, the goal is usually to find a balance between:
- keeping weight reasonably stable
- maintaining appropriate appetite control
- avoiding significant weight regain
- avoiding unnecessary further weight loss
- keeping treatment sustainable and well tolerated
At Pharmulous, our approach is therefore not:
“How high a dose can we reach?”
It is:
“What is the lowest effective and well tolerated treatment that helps this patient maintain their progress?”
What is Foundayo?
Foundayo contains orforglipron, a non-peptide GLP-1 receptor agonist taken as a tablet once daily.
Mounjaro contains tirzepatide and is taken as a once-weekly injection.
Although both can be used for weight management, they are different medicines. Tirzepatide acts on both GIP and GLP-1 receptors, whereas orforglipron acts on the GLP-1 receptor.
Foundayo should therefore not be thought of as simply “Mounjaro in tablet form.”
Is there evidence for using Foundayo after Mounjaro?
Yes.
The ATTAIN-MAINTAIN Phase 3b trial specifically studied people who had previously received injectable tirzepatide or semaglutide and had already achieved substantial weight loss.
Among participants who had reached a weight plateau following tirzepatide treatment, those receiving oral orforglipron maintained approximately 75% of their previous weight reduction after 52 weeks, compared with around 49% with placebo.
This is particularly relevant because it provides direct evidence that oral orforglipron can help preserve a significant proportion of weight previously lost with tirzepatide.
It does not, however, prove that Foundayo is equivalent to continuing Mounjaro. The trial did not include a group who simply remained on Mounjaro.
Why might someone consider Foundayo for maintenance?
Some patients will be perfectly happy remaining on Mounjaro.
Others may prefer a once-daily tablet rather than continuing weekly injections.
Foundayo may therefore be worth discussing if you have reached or are approaching your goal weight and want continued medication support, but your preferences or circumstances have changed.
Potential reasons may include:
- preferring tablets to injections
- a potentially more affordable weight management solution
- wanting a daily rather than weekly routine
- easier travel or storage
- needle anxiety
- looking for another long-term maintenance option
The important point is that maintenance does not have to look the same for everyone.
Do you need the highest Foundayo dose for maintenance?
No.
Foundayo is available in a range of strengths, with 17.2 mg being the maximum licensed daily dose.
However, higher doses are introduced according to treatment response and tolerability.
That does not mean every patient needs to reach the maximum dose.
During maintenance, a lower dose may be entirely appropriate if:
- weight is stable
- appetite remains appropriately controlled
- treatment is well tolerated
- you are not continuing to lose unwanted weight
The aim is not necessarily the highest dose - it is the right dose for you.
What if my BMI is now lower because Mounjaro worked?
This is an important maintenance question.
Someone who previously lived with obesity may now have a substantially lower BMI precisely because treatment has been successful.
A maintenance review should therefore consider the patient’s wider history rather than simply looking at one current BMI measurement.
At Pharmulous we consider:
- your starting BMI and weight
- how much weight you have lost
- your current BMI
- previous treatment
- appetite
- weight trajectory
- medical history
- risk of weight regain
- maintenance goals
We do not believe that a successful patient should automatically be viewed as though their previous weight history never existed.
How Pharmulous can help
At Pharmulous, we provide ongoing weight maintenance support for people who have reached or are approaching their target weight.
Our approach is:
Maintenance friendly - reaching your target does not automatically mean your treatment must stop.
Flexible - clinically appropriate treatment can be reviewed and adapted as your needs change.
Holistic - we consider the wider clinical picture rather than looking only at today’s BMI or a single number on the scales.
Patient centred - your treatment experience, preferences, lifestyle and long-term goals matter.
Clinically safe - flexibility does not mean compromising safety. All treatment decisions remain subject to appropriate clinical assessment.
Supported - if your appetite, weight, side effects or circumstances change, our clinical team is available to review your plan and provide support when needed.
Our clinicians look at your starting weight, current weight, treatment history, appetite, side effects, lifestyle and individual maintenance goals before recommending the most appropriate plan.
The goal is not simply to continue prescribing medication.
It is to help you find a safe, effective and sustainable approach to maintaining the progress you have worked hard to achieve.
Key takeaway
For selected patients, Foundayo may provide a useful oral option for weight maintenance after Mounjaro.
It will not be right for everyone, and switching treatments should always be clinically assessed.
But having another option allows maintenance to become more personalised - which is exactly what long-term weight management should be.
