Foundayo Maintenance Doses: Do You Need to Reach 17.2 mg?

When you start a weight-management medicine, it is easy to assume that success means progressing through every dose until you reach the maximum strength.

With Foundayo, that is not necessarily the case.

Foundayo comes in:

0.8 mg → 2.5 mg → 5.5 mg → 9 mg → 14.5 mg → 17.2 mg

Although 17.2 mg is the maximum dose, it is not necessarily the dose every patient needs to reach.

How does Foundayo dosing work?

For patients starting Foundayo according to the standard licensed pathway, (excluding dose adjustments in relation to established Glp-1 users), treatment begins at 0.8 mg once daily.

After at least 30 days, the dose is increased to 2.5 mg.

After at least another 30 days, it can progress to 5.5 mg.

Further increases to 9 mg, 14.5 mg and 17.2 mg may then be made after at least 30 days at the current dose according to the patient's treatment response and tolerability.

That wording matters.

It means dose escalation is not simply an automatic monthly race towards 17.2 mg.

Why are the doses increased gradually?

GLP-1 medicines can cause gastrointestinal effects such as nausea, vomiting, diarrhoea and constipation.

Gradual titration helps patients adjust to treatment and gives the clinician an opportunity to review effectiveness and tolerability before increasing again. The Foundayo product information specifically bases higher-dose escalation on response and tolerability.

What is a Foundayo maintenance dose?

There is not one universal Foundayo maintenance dose.

The most appropriate dose is the one that provides an acceptable balance between:

  • appetite control
  • weight stability
  • tolerability
  • your individual treatment goal.

For one patient this could potentially be 5.5 mg.

For another it may be 9 mg.

Others may require higher doses.

The number itself is less important than how the patient is actually responding.

Do I have to reach Foundayo 17.2 mg?

No.

17.2 mg is the maximum licensed daily dose.

If your current treatment is working well, weight is behaving as intended, appetite is appropriately controlled and side effects are manageable, increasing solely because a higher strength exists may not be necessary.

This is particularly important during weight maintenance.

Weight loss and maintenance have different goals

During active weight loss, treatment may be adjusted because further clinically appropriate weight reduction is still required.

During maintenance, the aim changes.

You may no longer want the scales to keep falling.

Instead, success may mean your weight stays within a relatively narrow range while you are able to:

  • eat as you would usually, paying particular attention to nutrition
  • manage hunger
  • maintain healthy habits
  • avoid significant regain
  • tolerate treatment comfortably

A stable weight during maintenance is not a plateau that necessarily needs “fixing”.

It may be exactly what treatment is intended to achieve.

What should be considered before increasing Foundayo?

Before assuming that you need a higher dose, consider the wider picture.

Is hunger genuinely increasing?

Has food noise returned?

Is weight consistently increasing over several weeks?

Are you still losing weight when you no longer want to?

Are you experiencing nausea, vomiting, constipation or difficulty eating enough?

Has anything else changed - activity, sleep, stress, medication or eating habits?

Looking at all of these factors is more useful than simply asking whether the next dose is available.

What if I'm doing well on a lower dose?

Then staying on that dose is always an option.

Pharmulous's weight maintenance approach describes a maintenance dose as the lowest effective dose for sustaining a stable, healthy weight at the patient's target, recognising that the right dose varies between individuals and may need adjustment over time.

This is particularly relevant with Foundayo.

Higher does not automatically mean better.

The goal should be appropriate treatment, not maximum treatment.

What if I'm continuing to lose too much weight?

That is also a reason for clinical review.

Once you have reached an appropriate maintenance weight, continuing to lose weight indefinitely is not necessarily desirable.

A clinician may need to consider:

  • your current dose
  • nutritional intake
  • appetite suppression
  • current BMI and previous weight
  • whether your maintenance target remains appropriate
  • whether treatment needs adjusting

Maintenance should support health, not simply produce the lowest possible number on the scales.

How Pharmulous can help

Pharmulous has been maintenance friendly from the outset because we recognise that weight management does not suddenly finish when a patient reaches their target.

Our maintenance model is intentionally flexible and patient centred.

We do not believe that every patient must automatically progress to the highest dose, nor do we believe that one maintenance strategy will suit everyone.

Instead, our clinicians consider the complete picture:

  • How is your weight behaving?
  • How is your appetite?
  • How are you tolerating treatment?
  • Where did you start?
  • Where are you now?
  • And what are you actually trying to achieve?

Our weight maintenance approach specifically supports ongoing medication, adjusted dosing and lifestyle guidance, with personalised plans reviewed by UK registered clinicians.

That gives patients the flexibility to have their treatment reviewed up or down where clinically appropriate, while keeping safety at the centre of prescribing.

If you are doing well, we do not want you to feel pressured to increase simply because a higher dose exists.

If treatment is becoming less effective, we can review that too.

And if you need help during maintenance, whether because of appetite changes, weight regain, unwanted continued weight loss or side effects — our clinical team remains available to support you.

Our approach is simple:

The lowest effective dose, personalised to the patient, with clinical support when it is needed.

Key takeaway

You do not automatically need to reach Foundayo 17.2 mg.

Foundayo dosing should be guided by your response, tolerability and treatment goals.

For maintenance patients in particular, the right dose is not necessarily the highest dose.

It is the dose that helps you maintain your progress safely, comfortably and sustainably.
