Wegovy Tablets and the Contraceptive Pill: Does Wegovy Make the Pill Less Effective?

If you're taking the contraceptive pill and considering Wegovy tablets, you may be worried that your weight loss medication could make your contraception less effective.

This is particularly understandable because warnings around contraception have been widely discussed with some other weight management medicines.

So what about oral Wegovy? Does semaglutide reduce absorption of the contraceptive pill?

Available interaction studies are reassuring.

The UK Wegovy product information reports no clinically relevant change in exposure to an oral contraceptive containing ethinylestradiol and levonorgestrel when used with semaglutide.

Therefore, semaglutide itself is not expected to reduce the effectiveness of this combined oral contraceptive through a clinically significant drug interaction.

However, there is another issue to consider.

What if Wegovy makes me vomit?

Wegovy can cause gastrointestinal side effects, particularly during dose escalation.

If you vomit after taking your contraceptive pill, the pill may not have had enough time to be absorbed.

For the combined pill, NHS advice says that if you vomit within 3 hours of taking most combined pills, or within 4 hours for some formulations such as Qlaira or Zoely,  another pill may be required.

Persistent vomiting or diarrhoea can also affect contraceptive protection.

What about the mini pill?

The advice varies depending on which progestogen-only pill you use.

The NHS advises that contraceptive protection can be affected if vomiting occurs within 2 hours of a traditional progestogen-only pill, or within around 3–4 hours for desogestrel or drospirenone pills. Severe watery diarrhoea may also affect protection.

So it is important to follow the instructions for your specific contraceptive pill.

How should I time my contraceptive pill with Wegovy tablets?

Wegovy itself should be taken first after an overnight fast.

Wait at least 30 minutes before taking other oral medicines, including your contraceptive pill if you normally take it in the morning.

You may find it simpler to continue taking your contraceptive at another established time of day, provided this is appropriate for your particular pill.

What if I have ongoing sickness?

Don't assume your contraception remains effective.

Check the instructions supplied with your contraceptive and speak to a pharmacist, GP or sexual-health clinician if you're uncertain.

If you have had unprotected sex while contraceptive protection may have been reduced, seek advice promptly because emergency contraception may sometimes be required.
