- Foundayo 17.2 mg: Do You Need the Highest...

Foundayo 17.2 mg is the maximum licensed daily dose of orforglipron.
But maximum does not mean mandatory.
For some patients, reaching 17.2 mg may be clinically appropriate.
For others, a lower dose may already be achieving exactly what treatment is intended to do.
So how do you know whether you actually need the highest Foundayo dose?
When can Foundayo increase to 17.2 mg?
Foundayo is gradually increased through:
0.8 mg → 2.5 mg → 5.5 mg → 9 mg → 14.5 mg → 17.2 mg
If you're currently taking the previous dose, read our guide to Foundayo 14.5 mg and when to consider moving up.
Patients should spend at least 30 days at each stage before another increase is considered.
For the higher strengths, escalation is specifically based on treatment response and tolerability.
17.2 mg is therefore the upper limit of the dosing pathway, rather than an automatic treatment target.
Who might need the maximum dose?
A higher dose may be considered where:
- further clinically appropriate weight loss is required
- appetite control remains inadequate
- lower doses have been well tolerated
- the clinician believes additional treatment effect is appropriate
The decision should always reflect the individual patient's circumstances.
Does 17.2 mg cause more weight loss?
Clinical trial data demonstrate a dose-response relationship, with higher studied doses generally producing greater average weight reduction.
But averages do not determine the best dose for one individual patient.
Higher doses can also increase treatment exposure and may be associated with greater gastrointestinal tolerability issues.
The aim is therefore not simply to maximise weight loss at any cost.
When might you not need 17.2 mg?
You may not necessarily require the maximum dose if:
- your current dose is working well
- appetite remains controlled
- weight loss is progressing appropriately
- you have reached your maintenance goal
- further weight loss is not desirable
- increasing could worsen troublesome side effects
This is why medication strength should always be considered alongside the patient's clinical goal.
What about Foundayo 17.2 mg during maintenance?
For someone in weight maintenance, the highest dose may or may not be appropriate.
Maintenance is not about continuing to push the scales down indefinitely.
It is about finding a sustainable balance between:
- stable weight
- manageable appetite
- adequate nutrition
- acceptable side effects
- long-term health
If a lower dose achieves this, increasing simply to reach 17.2 mg may offer little benefit.

What if my weight has plateaued?
A plateau alone does not automatically mean your dose needs increasing.
Before recommending a change, clinicians may consider:
- how long the plateau has lasted
- food intake
- activity
- resistance exercise
- sleep
- appetite
- medication
- hormonal or medical factors
- how much weight has already been lost
Weight loss naturally becomes more difficult as body weight falls.
Medication is only one part of the wider picture.
How Pharmulous can help
At Pharmulous, we believe higher is not automatically better.
Our prescribing approach is designed to remain:
Maintenance friendly — patients who reach their goal are supported rather than automatically taken off treatment.
Flexible — doses can be reviewed as your needs change.
Holistic — we look at appetite, nutrition, activity, weight history, side effects and wider health rather than focusing only on a number on the scales.
Patient centred — your experience and treatment goals matter.
Clinically safe — dose escalation is only considered where appropriate and tolerability remains an important part of every decision.
Supported — if your treatment is not doing what you expect, you can ask our clinical team to review it rather than trying to manage dose changes yourself.
We do not believe every patient needs 17.2 mg.
We believe every patient deserves a treatment plan that is appropriate for them.
The maximum dose is an option — not a destination.
