Foundayo Doses Explained: 0.8 mg to 17.2 mg

Foundayo (orforglipron) is a once-daily oral GLP-1 medicine used for weight management in eligible adults.
If you are starting Foundayo, you may have noticed that there are six different strengths:
0.8 mg → 2.5 mg → 5.5 mg → 9 mg → 14.5 mg → 17.2 mg
But what does each dose actually mean? How quickly should Foundayo be increased? And does everyone eventually need to reach 17.2 mg?
The short answer is no.
Foundayo treatment begins at a low dose and is increased gradually. Once patients reach the higher strengths, further increases are based on treatment response and tolerability, rather than simply moving everyone automatically to the maximum dose.
What is the Foundayo dosing schedule?
The standard licensed schedule begins with Foundayo 0.8 mg once daily.
After at least 30 days:
0.8 mg → 2.5 mg
After at least another 30 days:
2.5 mg → 5.5 mg
Higher doses can then be considered:
5.5 mg → 9 mg → 14.5 mg → 17.2 mg
with at least 30 days at each dose before another increase.
Importantly, increases beyond this stage are based on how the patient is responding and tolerating treatment. The maximum licensed dose is 17.2 mg once daily.
Foundayo 0.8 mg: The Starting Dose
Foundayo 0.8 mg is the standard starting dose.
If you're just starting treatment, read more about what to expect during your first month on Foundayo 0.8 mg.
For someone who is new to treatment, this introductory dose allows the body to become accustomed to orforglipron gradually.
Patients normally take 0.8 mg once daily for at least 30 days before progressing to 2.5 mg.
It is important not to judge your eventual response to Foundayo entirely by your first few weeks.
The purpose of an introductory dose is partly tolerability and safe dose escalation, rather than immediately producing the maximum treatment effect.
Foundayo 2.5 mg: What Happens Next?
After at least 30 days on 0.8 mg, the licensed schedule moves to Foundayo 2.5 mg once daily.
This is another step in gradually introducing the medicine.
During this stage, your clinician may consider:
- appetite and hunger
- food cravings
- weight trajectory
- nausea or digestive symptoms
- constipation or diarrhoea
- overall tolerability
The aim is not simply to progress through the doses as quickly as possible.
Foundayo should be increased according to the licensed schedule and your individual clinical response.
Foundayo 5.5 mg: Can You Stay on This Dose?
Foundayo 5.5 mg is an important point in the dosing pathway.
After at least 30 days on 2.5 mg, the dose can be increased to 5.5 mg. From here, further increases to 9 mg, 14.5 mg and 17.2 mg are considered according to response and tolerability.
That means a patient doing well on 5.5 mg does not necessarily need to think of it simply as a stepping stone towards 17.2 mg.
For example, remaining on a lower dose may be worth discussing if:
- appetite is appropriately controlled
- weight is responding as intended
- side effects are minimal
- you are moving into weight maintenance
- further weight loss is not required
The right decision depends on the individual.
Foundayo 9 mg: When Is It Appropriate?
Foundayo 9 mg is one of the higher treatment doses.
Patients may move to 9 mg after spending at least 30 days on 5.5 mg where an increase is clinically appropriate.
Foundayo 9 mg is also relevant to some patients switching from Mounjaro to Foundayo or from other established injectable GLP-1 treatment.
The Foundayo product information confirms that the 9 mg marketed tablet provides equivalent exposure to the 12 mg investigational capsule formulation used in clinical studies.
However, this should not be interpreted as a general switching rule.
There is no manufacturer-approved milligram-for-milligram conversion between Foundayo and medicines such as Mounjaro.
Previous treatment, tolerability and any treatment gap need to be assessed individually.
Foundayo 14.5 mg: Should You Increase?
Foundayo 14.5 mg is the second highest available strength.
Someone may progress from 9 mg to 14.5 mg if:
- their current treatment response is insufficient
- the previous dose has been adequately tolerated
- further weight management is clinically appropriate
But increasing is not automatically necessary simply because the patient has completed 30 days on 9 mg.
The product information specifically states that higher dose escalation is based on treatment response and tolerability.
This distinction becomes particularly important during maintenance.
Foundayo 17.2 mg: The Maximum Dose
Foundayo 17.2 mg is the maximum licensed daily dose.
If you are approaching maintenance, you can read more about Foundayo maintenance doses and whether you need to reach 17.2 mg.
It is not necessarily the final destination for every patient.
Some people may require the maximum strength to achieve an appropriate response.
Others may achieve their treatment goals at a lower dose.
The question should therefore not simply be:
“When can I increase to 17.2 mg?”
A more useful question is:
“Do I actually need to increase?”
Does a higher Foundayo dose mean better results?
Clinical studies generally demonstrated greater average weight reduction at higher orforglipron exposures.
However, treatment decisions are not based on weight loss alone.
Higher exposure can also mean a greater likelihood of gastrointestinal side effects.
The appropriate dose therefore needs to balance:
- Effectiveness
- Tolerability
- Treatment goal
rather than simply choosing the strongest available tablet.
Do I need to increase Foundayo every month?
Not indefinitely.
Patients need to remain on each dose for at least 30 days before moving to the next strength.
But after reaching the higher treatment stages, whether to increase further depends on response and tolerability.
There may therefore be circumstances where remaining on the same dose for longer is appropriate.
Never increase Foundayo yourself without discussing this with your prescriber.
What about Foundayo for weight maintenance?
Maintenance has a different goal from active weight loss.
Once you have reached an appropriate weight, success may mean:
- keeping weight broadly stable
- maintaining manageable hunger
- reducing problematic food cravings
- maintaining good nutrition
- avoiding unwanted further weight loss
- keeping side effects minimal
A dose that successfully achieves these things may be doing exactly what is required.
A stable weight is not necessarily a plateau that needs to be “fixed” with a higher dose.
How Pharmulous can help
At Pharmulous, we do not see Foundayo treatment as a race through six doses towards 17.2 mg.
Our approach is maintenance friendly, flexible and patient centred, while always remaining clinically safe.
Before recommending a change, our clinicians consider:
- your weight trajectory
- appetite and food noise
- how much weight you have already lost
- previous GLP-1 treatment
- current side effects
- treatment tolerability
- your current BMI and previous weight history
- whether you are actively losing weight or maintaining
- your individual goals
If your current dose is working well, we do not want you to feel pressured to increase simply because another strength exists.
Equally, if appetite control is reducing or treatment is no longer achieving its intended purpose, your dose can be reviewed.
Our aim is to find the lowest effective, well-tolerated dose that safely supports your individual treatment goals – and it always has been.

Do I have to reach 17.2 mg?
No. 17.2 mg is the maximum dose, not an automatic target for every patient.
Key Takeaway
Foundayo comes in six strengths:
0.8 mg, 2.5 mg, 5.5 mg, 9 mg, 14.5 mg and 17.2 mg.
But treatment is not simply about progressing through every strength.
The best Foundayo dose is the one that provides an appropriate balance between effectiveness, tolerability, safety and your individual goals.
At Pharmulous, we believe in treating the patient rather than simply following a dose ladder.
