Foundayo 9 mg: When Is It Used and Do You Need to Increase?

Foundayo 9 mg is the first of the higher Foundayo dose levels reached after the initial titration stages.
For some patients, it may provide an effective balance between appetite control, weight response and tolerability.
For others, a later increase to 14.5 mg may be appropriate.
The important point is that moving beyond 9 mg is not automatic.
How do you reach Foundayo 9 mg?
The standard Foundayo schedule is:
0.8 mg → 2.5 mg → 5.5 mg → 9 mg → 14.5 mg → 17.2 mg
Patients remain at each dose for at least 30 days.
Once a patient has completed at least 30 days on Foundayo 5.5 mg, the dose may increase to 9 mg according to treatment response and tolerability.
That word — may — is important.
By this stage, dosing becomes increasingly individualised.
What can I expect from Foundayo 9 mg?
Some patients may notice stronger effects on:
- appetite
- hunger
- food cravings
- portion sizes
- fullness
- weight trajectory
However, the response varies considerably between individuals.
A higher numerical dose is not automatically the best dose.
The goal is to achieve enough treatment effect while maintaining acceptable tolerability.
Can I stay on Foundayo 9 mg?
Potentially, yes.
If Foundayo 9 mg is:
- controlling appetite appropriately
- supporting the desired weight outcome
- well tolerated
- helping you maintain your weight
there may not always be a reason to increase immediately.
The UK SmPC specifically states that progression from 5.5 mg through the higher strengths should be based on response and tolerability.
When might 14.5 mg be considered?
Moving from 9 mg to 14.5 mg may be considered where:
- hunger has become difficult to manage
- weight response is inadequate during active weight loss
- food noise or cravings are returning
- 9 mg has been well tolerated
- further treatment effect remains clinically appropriate
Patients should remain on 9 mg for at least 30 days before any increase.
Is Foundayo 9 mg important when switching from Mounjaro?
It can be.
Clinical development used an earlier orforglipron capsule formulation. The UK SmPC confirms that the marketed Foundayo 9 mg tablet provides equivalent exposure to the investigational 12 mg capsule used in those studies.
This is relevant to some of the clinical evidence around switching from Mounjaro to Foundayo.
However, it does not create a universal Mounjaro-to-Foundayo conversion.
There is no simple milligram-for-milligram equivalence between tirzepatide and orforglipron.
Any switching dose needs to consider previous treatment, tolerability and the treatment gap.
What about Foundayo 9 mg for maintenance?
For some patients during weight maintenance, Foundayo 9 mg may be sufficient.
During maintenance, the aim may be:
- stable weight
- manageable hunger
- reduced food noise
- good nutritional intake
- acceptable side effects
If those goals are being achieved on 9 mg, continuing upward solely because 14.5 mg and 17.2 mg exist may not be necessary.
Maintenance is about finding an effective long-term balance rather than continuously escalating treatment.
Important clinical note:
The UK SmPC states that 9 mg is the maximum Foundayo dose when used with certain strong CYP3A4 inhibitors or clinical OATP1B inhibitors.
This is one reason your prescriber needs an up to date list of all medicines you take before increasing Foundayo.
Do not change doses yourself.
What if 9 mg is causing side effects?
Side effects should form part of every dosing decision.
If you are experiencing ongoing:
- nausea
- vomiting
- constipation
- diarrhoea
- difficulty eating
- significant abdominal discomfort
- a higher dose may not be appropriate simply because you have completed 30 days.
Speak to your prescriber before escalating.
How Pharmulous can help
At Pharmulous, we take a patient-centred rather than dose-centred approach.
When reviewing Foundayo 9 mg, our clinicians consider:
- how well the dose is controlling appetite
- current weight trajectory
- food noise and cravings
- side effects
- how much weight you have already lost
- whether you are actively losing or maintaining
- previous GLP-1 treatment
- other medication and relevant medical conditions
- your individual treatment goals
We are maintenance friendly, so patients who are doing well do not need to feel that they must continuously chase the next strength.
Our treatment plans remain flexible where clinically appropriate, but safety always comes first.
If 9 mg is working, that matters.
If it stops working adequately, that matters too.
And if your needs change, our clinical team can review the treatment with you.
Frequently Asked Questions
Is Foundayo 9 mg a high dose?
It is one of the higher Foundayo treatment strengths, although 14.5 mg and 17.2 mg are also available.
Can I stay on Foundayo 9 mg?
Potentially, if your response and tolerability remain appropriate.
How long should I take 9 mg before increasing?
At least 30 days before moving to 14.5 mg.
Do I need to increase if my weight is stable?
Not necessarily, particularly if weight stability is your maintenance goal.
Is Foundayo 9 mg equivalent to Mounjaro 10 mg or 15 mg?
No. There is no direct dose to dose conversion between the medicines.
Key Takeaway
Foundayo 9 mg is an important treatment dose, but it does not automatically need to lead to 14.5 mg.
The decision should be based on response, tolerability and treatment goals.
At Pharmulous, we focus on finding the dose that works safely for the individual patient rather than progressing automatically because a higher dose exists.
